Application to Marry and Required Premarital Statement Form provided by Married by Mail will need to be printed.

One spouse will need to sign the Application  in box 9 – the other spouse will need to sign the Application in box 13, respectively in black ink only.

The signed Application to Marry will need to be sent to Married by Mail along with copies of couples ID’s (Passports or Drivers Licenses) as well as the signed, dated Required Premarital Statement Form to:
